Seizures in labor

Up to five percent of women with epilepsy will have a tonic-clonic seizure during labor or the puerperium. In women with no prior history of epilepsy, eclampsia is the most common cause of seizures in labor. If tonic-clonic seizures occur during labor, maternal hypoxia, fetal hypoxia and acidosis will result. Status epilepticus in labor is associated with increased maternal as well as fetal mortality. Seizures in labor should be terminated as soon as possible using intravenous lorazepam or diazepam. If seizures persist, manage as for status epilepticus
