Description

This Continuing Medical Education (CME) session will focus on the comprehensive surgical management of patients requiring total gastrectomy, a major operative procedure commonly performed for advanced gastric malignancies, diffuse gastric cancer, and selected benign conditions. The session will review the clinical indications, patient selection criteria, and preoperative evaluation necessary to optimize surgical outcomes. Participants will examine key surgical principles including operative approaches, lymph node dissection, reconstruction techniques (such as Roux-en-Y esophagojejunostomy), and intraoperative decision-making. The discussion will also address potential complications including anastomotic leak, nutritional deficiencies, dumping syndrome, and long-term metabolic consequences. Emphasis will be placed on multidisciplinary perioperative care, enhanced recovery strategies, nutritional rehabilitation, and long-term follow-up of patients after gastrectomy. Case-based discussions will be used to highlight practical challenges and best practices in surgical oncology.

Objectives

By the end of this CME session, participants will be able to: Describe the common indications for total gastrectomy, particularly in gastric malignancies and selected benign conditions. Outline the preoperative evaluation and staging workup required before undertaking total gastrectomy. Explain the key steps of the surgical procedure, including gastric resection, lymphadenectomy, and reconstruction techniques. Recognize potential intraoperative and postoperative complications associated with total gastrectomy. Implement appropriate postoperative management strategies, including pain control, early mobilization, and nutritional support. Identify long-term metabolic and nutritional consequences, including vitamin B12 deficiency, iron deficiency, and weight loss. Apply multidisciplinary approaches involving surgeons, oncologists, nutritionists, and nursing teams to optimize patient outcomes. Discuss strategies for surveillance and long-term follow-up after total gastrectomy.
