Description
Effective communication among healthcare professionals is essential for continuity of care, clinical decision-making and patient safety. Communication breakdowns during patient handovers, referrals and clinical escalations can contribute to medical errors, treatment delays and adverse patient outcomes. This CME/Workshop will introduce and explore the ISBAR (Identification, Situation, Background, Assessment and Recommendation) communication framework as a standardized approach to clinical communication.
Objectives
By the end of the session, participants should be able to:
Explain the importance of effective communication in promoting quality healthcare and patient safety.
Describe the five components of the ISBAR communication framework.
Identify common communication barriers and their implications for patient care.
Demonstrate the application of ISBAR during clinical handovers, referrals and escalation of care.
Evaluate how standardized communication tools can improve teamwork, accountability and continuity of care.
