Description
NECROTIZING ENTEROCOLITIS Definition: An ischaemic and inflammatory necrosis of the bowel primarily affecting (premature) neonates after initiation of enteral feeding Incidence: 4-10% infants weighing <1500gms, with the highest incidence in the most premature infants 10% NEC cases occur in term infants (most with pre-existing conditions)
Objectives
Term- usually diagnosed in the 1st week of life
Preterm- >14days old or at 30-32 weeks PMA
Feeding intolerance
Increased gastric residuals (aspirates)
Blood in stools
Abdominal distension and tenderness
Abdominal skin discolouration
Bilious NGT drainage
Systemic symptoms- non-specific including apnoea, bradycardia; temp instability, hypotension, circulatory shock
Mild (medical Mx) to severe (7%; fulminant with rapid progression to NEC totalis, septic shock, severe metabolic acidosis and death)
